We provide NHS physiotherapy under the AQP (Any Qualified Provider) service.

This service allows patients who have been referred for physiotherapy by their GP or consultant to choose where they receive their treatment.

To access this service, you must have an NHS referral.

How to Refer

Swale GP referrals are received in the following ways:

  • Sent via email using a referral form
  • Given directly to the patient to contact us
  • Through the NHS e-Referral system (Choose & Book)

What We Treat

This service includes, but is not limited to:

We support patients with:

  • Neck and back problems (including whiplash, sciatica, and ongoing back pain)
  • Tennis elbow and related conditions
  • Frozen shoulder
  • Tendon problems in the upper limb
  • Sports injuries
  • Pre and post-natal patients (including treatment after 35 weeks)
  • Post-operative patients who require physiotherapy in the community
  • Patients seen in A&E who do not require fracture clinic follow-up

Eligibility

Patients suitable for this service include:

  • Patients aged 16 and over
  • Patients registered with a Swale GP
  • Patients referred by a GP, consultant, or healthcare professional

Exclusions

This service does not include:

  • Patients aged 16 and over
  • Patients with suspected serious pathology or red flag symptoms
  • Housebound patients
  • Patients not registered with a Swale GP
  • Conditions not related to musculoskeletal problems
  • Patients currently receiving physiotherapy as part of hospital care

Appointments

Once we receive your referral, it will be processed within 3 days.
Appointments are usually offered within 2 weeks.

DNAs and Cancellations

Under NHS AQP guidelines:

One missed appointment (DNA) is allowed
A further appointment may be offered if you contact us within 24 hours
If not, you may be discharged back to your GP
